Job description
Field-based case manager conducting in-home visits for members enrolled in Long-Term Services and Support (LTSS) programs in the Fort Worth/Tarrant county area of Texas. Responsible for comprehensive assessments, care planning, provider collaboration, and coordinating services with members, PCPs, and care teams. Schedules and attends interdisciplinary meetings, advocates for safe discharge, authorizes services within LTSS benefit, and documents in the EHR. Uses motivational interviewing and coaching to engage members.

What you bring
- Active TX RN license
- 2+ years clinical practice experience
- Reliable transportation
