Job description

Field-based RN case manager conducting in-home assessments for members in Managed Long-Term Services and Supports (MLTSS) and Dual Special Needs Programs (D-SNP/FIDE) across Mercer County NJ. Up to 75% field travel, M-F 8-5 EST. Develop individualized care plans coordinating physical, behavioral, and psychosocial services. Evaluate needs and coordinate adult/pediatric medical day care, personal care assistant services, nursing facility placement, and MLTSS enrollment. Partner with members, caregivers, PCPs, and interdisciplinary teams. Authorize services per program guidelines and support transitions of care. Document in EHR. Requires active NJ RN license, 3+ years clinical experience, 3+ years in home health/hospice/similar, ability to manage pediatric and medically complex cases, residency in Mercer County NJ, NJ Choice Certification, and reliable transportation.
