Job description
Field-based case manager role with Aetna serving dual-eligible (Medicare/Medicaid) members in the South Loop/Chinatown area of Chicago. Uses a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy to meet members' comprehensive health and social-determinant needs. Interacts with members telephonically and in person, including home visits, worksites, and physician offices. Travel up to 75% required.
Key responsibilities
- Assess members' medical and vocational status and develop plans of care
- Coordinate case management activities across the continuum of care
- Conduct in-person and telephonic member visits
- Liaise with providers, family, employers, and community resources
- Document case work in compliance with regulatory and company policies
What you bring
- Active Illinois RN license
- 3-5 years clinical experience
- 2-3 years case management/discharge planning/home health experience
- Reliable transportation, willing to travel 75%
- Bilingual Cantonese/Mandarin preferred
- CCM preferred
