Job description
CVS Health is hiring a Case Manager RN to coordinate care for members telephonically. You will assess member needs, develop care plans, and collaborate with providers to ensure optimal outcomes. This is a 100% remote role requiring an active compact RN license and 3+ years of clinical experience. Salary range: $70,000–$90,000.
Key responsibilities
- Conduct telephonic assessments of member health needs

- Document all interactions in the care management system
