Case Management Coordinator (Pima County, Arizona) ($5000 sign-on bonus)

The Case Management Coordinator is a work from home position with travel required (in Pima County, Arizona) to visit members. Utilizes skills to coordinate, document, and communicate all aspects of the utilization/benefit management program. Applies critical thinking and knowledge in clinically appropriate treatment, evidence-based care, and medical necessity criteria for members by providing care coordination, support, and education through the use of care management tools and resources. Duties include evaluation of members through comprehensive evaluation of referred member’s needs/eligibility, identifying high-risk factors, coordinating and implementing care plan activities, monitoring care plan progress. Enhances medical appropriateness and quality of care using a holistic approach, presents cases at case conferences, utilizes motivational interviewing skills to engage members. Monitors, evaluates, and documents care in compliance with regulatory and accreditation guidelines. Requires 2+ years of experience in case management, working with people with serious mental illness (SMI) and elderly or physically disabled. Must reside in Pima County, Arizona and travel up to 50% within the county. Bachelor’s degree or licensed RN (Registered Nurse) valid in Arizona required.
