Job description
Care Manager RN on a Special Needs Plan (SNP) care team, coordinating care for members with multiple chronic medical and behavioral health conditions and social determinants of health needs. 50-75% of the day is dedicated to telephonic engagement with members and care coordination. Conduct comprehensive assessments, develop individualized care plans with goals and interventions, and provide evidence-based disease management education. Collaborate with the interdisciplinary care team to address barriers to care, connect members with health plan benefits and community resources, and ensure regulatory timelines are met. Document care management activity in the member's EHR with accuracy and timeliness. Support other Care Team members through clinical decision-making and guidance. Required: active unrestricted RN license in Colorado OR compact license in state of residence; minimum 3+ years nursing experience; minimum 2+ years case management, discharge planning, or home healthcare coordination experience; ASN required, BSN preferred. Preferred: Medicare/Medicaid care management experience, work with SDoH and behavioral health populations, bilingual English-Spanish. Salary range $60,522–$129,615 plus bonus eligibility.

What you bring
- Active unrestricted RN license (CO or compact)
- 3+ years nursing experience
- 2+ years case management, discharge planning, or home health coordination
- Microsoft Office Suite proficiency
- Knowledge of Medicare/Medicaid managed care (preferred)
- SDoH and behavioral health experience (preferred)
- Bilingual English-Spanish (preferred)
