Care Manager, Registered Nurse - Transition of Care, Physical Health

The Care Manager, RN for Transition of Care (Physical Health) works telephonically to support members transitioning from inpatient to outpatient settings. Responsibilities include assessing member needs, coordinating care, educating on medications and follow-up appointments, and collaborating with providers and community resources to ensure smooth transitions and reduce readmissions. Requires active RN license (compact preferred), 3+ years clinical experience, and strong communication skills. BSN preferred. Salary range $70,000-$90,000.
