Job description
The Care Manager RN uses clinical expertise to coordinate care for members with complex health needs. This is a telephonic role working from home, assessing member needs, developing care plans, and collaborating with providers and community resources. Requires active RN license (compact preferred) and 3+ years of clinical experience. BSN preferred. Salary range: $70,000–$90,000.
