Job description
The Care Manager RN is part of CVS Health's Special Needs Plan (SNP) care team, coordinating care for members with multiple chronic medical and behavioral health conditions and social determinants of health (SDoH) needs. 50-75% of the day is spent on telephonic engagement with members. Responsibilities include conducting comprehensive assessments, developing individualized care plans with goals and interventions, providing evidence-based disease management education, and connecting members with health plan benefits and community resources. The Care Manager collaborates with the interdisciplinary care team to address barriers to care, advocates for needed services, and supports other Care Team members with clinical decision-making and guidance. Meticulous and timely documentation in the member's EHR is essential, as is meeting regulatory requirements and productivity metrics including call volume and member engagement. Required: active unrestricted Compact RN license in state of residence, ASN with relevant healthcare experience, 3+ years nursing experience, 2+ years case management/discharge planning/home healthcare coordination experience, proficiency in Microsoft Office Suite, and a private dedicated workspace. Preferred: BSN, Medicare/Medicaid care management experience, experience with SDoH and behavioral health populations, and English-Spanish bilingual skills. Salary range $60,522–$129,615 plus bonus eligibility.
