Job description
The Care Coach (LTSS) engages members telephonically to support long-term services and needs. Assesses member needs, develops care plans, coordinates community resources, and monitors outcomes. Requires active RN license, 2+ years clinical experience, and knowledge of LTSS. Compact license accepted. Salary range $65,000-$85,000.
